Service Request Form

The Service Request Form provides on-line submission for services or problem notifications directly to the Township. The routing to the proper department is done automatically based on the type of request and a tracking number is automatically generated. Please be as specific as possible. More types of requests will be available in the near future.

Non-Emergency Requests only! This is not to be used for 9-1-1 type emergency requests. Please contact the Police at (973) 697-1300 if you need an immediate response.
