1.      Name of person or entity responsible for conducting the event.  If the entity is a charity, partnership or corporation, list the name of the president, vice president, secretary, treasurer, directors, trustees and/or partners of said entity. (Attach additional information if necessary)

7.      Attach a map or sketch showing the area sought to be licensed, delineating the parking of automobiles, showing driveways or means of ingress to and egress from said premises and the location of all sanitary facilities.  Parking and Traffic Control Plan according to 67-7 (C) of the ordinance.

8.      Names and address of property owners adjoining premises on which event will be conducted or area to be used for parking purposes.

PLEASE TAKE NOTICE that pursuant to Ordinance 28-98, the Township Council either prior to granting any license for an outdoor gathering or as a condition to the issuance of a license, in its discretion, may require any applicant to notify property owners within a designated distance from the property or properties upon which the outdoor gathering is being held to either provide said surrounding property owners with an opportunity to be heard considering whether or not to grant or deny the application and/or to notify surrounding property owners that an event or events will be conducted at certain given times.  Attachments:  1) Chapter 67 – Outdoor Gatherings, Licensing And Regulations; 2) Permit Application – Bureau of Fire Prevention
